    The Hospice Assistant Bereavement Coordinator is a professional who collaborates with and assists the Hospice Bereavement Coordinator with the overall Hospice and Home Based Service Bereavement needs. The Hospice Assistant Bereavement Coordinator facilitates the supportive administrative bereavement duties to assist to meet the needs of the patient and caregivers, with a holistic approach, keeping with the patient and caregiver's beliefs and goals of care. The Hospice Assistant Bereavement Coordinator coordinates with the Hospice IDG members, Home Based Service members, patients, caregivers, providers, Medical Director, referral sources, community sources and the Hospice IDG members to work toward plan of care goals. The Hospice Assistant Bereavement Coordinator co-facilitates support groups, conducts bereavement calls and provides bereavement support to patients and families under the supervision and guidance of Hospice Bereavement Coordinator. The Hospice Assistant Bereavement Coordinator documents in the patient's medical records in a timely manner according to agency requirements. The Hospice Bereavement Assistant Coordinator provides patient care based on provider's orders, departmental policies and procedures, regulatory guidelines and best practice. The Hospice Bereavement Assistant attends and actively participates in IDG and team meetings.

  • Required Education:
    Bachelor's Degree in Social Work, Psychology, related field.

    Required Technical Skills:
    Generally accepted standards of practice in respective discipline. Professional and efficient verbal and written communication skills. Good interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.) Ability to be open, sensitive, flexible and problem solve.

    Preferred Technical Skills:
    Have demonstrated experience in working with patients and caregivers dealing with life-threatening illness and end of life. Experience with Home Care Home Base; Experience with smart phones and smart devices. Microsoft Office.

    Required Licensure /Registration /Certification:
    Current Texas State license; Current auto insurance. Must have reliable transportation.

    Preferred Licensure /Registration /Certification:
    Advanced education or certification in area of specialty

    Required Experience:
    One year experience in community or healthcare. Experience in administrative assistant duties.

    Preferred Experience:
    Experience in respective discipline in a hospital or other healthcare setting; Hospice or Home Care experience; Experience in mental health; Experience with bereavement services and programs.
